چکیده

OBJECTIVE: To quantify variation of mean paediatric patient length of stay (LoS) among intensive care units (ICUs) in Australia and New Zealand METHODS: Retrospective data from Australian and New Zealand institutions with paediatric ICU admissions were analysed. The data were collected between 1997 and 2006 providing a total of 123 institution years of data (an average of 6.15 years per site). Patient LoS was modelled as the outcome variable of a gamma regression with patient risk factors entered as fixed effects (allowing adjustment for case mix) and variation among ICUs modelled using a random effect. RESULTS: Six Australasian ICUs had an average risk-adjusted paediatric LoS which was significantly shorter than average, while five had an average LoS that was significantly longer than average. The remaining nine sites had average LoS that were not significantly different from the average (at the 95% level). Among other risk factors, previous admission to an ICU and respiratory support within the first hour of admission were both associated with prolonged LoS. CONCLUSION: There was significant variation in paediatric patient LoS at the ICU level not accounted for by patient case mix. This has important implications for efficiency of ICU processes and, possibly other parameters of quality of patient care in those institutions with longer LoS.

Blood transfusion practice indicated by paediatric intensive care specialists in response to four clinical scenarios.

OBJECTIVE Several clinical guidelines exist for blood transfusion in adults but only one refers to children. There are no guidelines for blood transfusion in critically ill children and the paediatric intensive care specialist's approach is unknown. We wished to evaluate the approach toward blood transfusion among a group of paediatric intensive care specialists. METHODS All certified paediat...

Design and Implementation of a Fuzzy Intelligent System for Predicting Mortality in Trauma Patients in the Intensive Care Unit

Introduction: The intensive care unit is one of the most costly parts of the national health sector. These costs are largely attributable to the length of stay in the intensive care unit. For this reason, there are significant benefits in predicting patients' length of stay and the percentage of deaths in intensive care units. Therefore, in this study, a fuzzy logic based intelligent system was...

Correlation between Quality of Care and Length of Hospital Stay in Neonatal Intensive Care Unit

Background: Length of hospital stay (LHS) is the most important and practical indicator in hospitals, which largely reflects the level of hospital performance and activity. It is also an important indicator for resource planning. Moreover, the quality of care can theoretically be related to LHS. The aim of this study was to investigate the relationship between LHS and quality of nursing care in...
